Individually Identifiable Health Information (IIHI)

In the context of the Health Insurance Portability and Accountability Act of 1996, information that:

• Relates to:

o an individual's past, present, or future physical or mental health or condition;

o an individual's provision of health care; or

o past, present, or future payment for the provision of health care to an individual.

• Identifies the individual or for which there is a reasonable basis of belief that it can be used to identify an individual.

• Is created or received by a health care provider, health plan, employer or health care clearinghouse.
