A questionnaire prescribed under section 74 of the Sex Discrimination Act 1975 that those who believe they may have been discriminated against on the grounds of their sex or marital status can serve on the alleged discriminator. The aim is to get information about the treatment in question or the employer's general treatment of women or men in order to decide whether or not to bring legal proceedings before an employment tribunal; and if proceedings are brought, to present their complaint in the most effective way.
